Summary

The Care Manager I / CCBHC Care Coordinator II will provide case coordination, outreach, and advocacy services to individuals entering CCBHC. They will also provide initial screening and referral services to help the consumer connect to needed resources, assist in developing psychosocial evaluations, conduct health screenings, and develop coordinated treatment plans.


Essential Duties / Responsibilities

* Consistently exercising discretion and judgment to analyze situations and decide necessary actions based on each case.
* Working without daily supervision, evaluating possible courses of conduct and making decisions independently.
* Maintaining an active caseload, providing referrals and linkages within expertise and credentials, and procuring additional services as needed.
* Acquiring and providing detailed consumer information to establish the foundation for the Treatment Plan.
* Working with the consumer daily using professional judgment to implement the team‑determined Treatment Plan.
* Assisting in the development, implementation, and revision of individual treatment plans; ensuring services specified in the plan are provided and monitoring progress toward treatment goals.
* Communicating consistently with community partners to maintain positive working relationships for existing MOUs.
* Consulting and coordinating with community systems to facilitate linkage, referral, crisis management, advocacy, and follow‑up with a focus on attaining treatment goals.
* Providing crisis management for consumers and making linkages for appropriate interventions.
* Providing individual and group psychosocial rehabilitative services.
* Providing face‑to‑face skill training and mental health interventions in accordance with the treatment plan.
* Providing age‑appropriate life skills training to consumers individually or in groups.
* Maintaining consumer and program records in accordance with applicable standards, regulations, and grant requirements.
* Maintaining a high level of ethical conduct regarding confidentiality, dual relationships, and professional stature.
* Demonstrating competent use of Electronic Medical Record (EMR).
* Participating in continuing education activities to remain knowledgeable in the area(s) of expertise.
* Attending meetings as appropriate and meeting regularly with supervisors to exchange information and receive supervision.
* Completing CANS and/or ANSA training and ongoing certification.
* Timely completing DARMHA Supported Consumer (DSC) paperwork and reassessments within specified deadlines.
* OBHP qualified staff to participate in required clinical supervision.
* Maintaining a flexible schedule in collaboration with supervisor.
* Performing other duties as assigned.


Minimum Qualifications

Care Manager I

* High school diploma or equivalent.
* At least 12 months of paid, full‑time employment or 24 months of paid, part‑time employment in a directly related adult, child, youth, adolescent or family services field (e.g., Community Mental Health Center, Group Home, Residential Placement facilities, Personal Caregiver, State Hospital, Certified Nursing Assistant, Licensed Professional Nurse, Department of Children’s Services, Family and Social Services Administration, Education/Special Education, Nursing Home, Day Care, Juvenile Justice Centers, Community Corrections/Probation, FQHC, Hospitals, Hospice).

CCBHC Care Coordinator II

* Bachelor’s degree in psychology, social work, sociology, family/consumer sciences, child development, substance abuse, mental health, human services, education, criminology, counseling, nursing, special education, family studies, marriage and family therapy, recreational therapy, music therapy, art therapy, or related field with a minimum of 1 year of case management/care management experience.
* Nonspecific bachelor’s degrees with a minimum of 4 years of experience as a Care Coordinator I in a related child, youth, adolescent, or family services field.
* Completion of case management training post‑college degree.


All Positions Must Possess

* Knowledge and skills in community‑based behavioral health care and case management.
* Good interpersonal and communication skills.
* Agreement to use personal transportation in work capacity, including transporting consumers.
* Flexible scheduling.
* Willingness to use reliable personal transportation for work capacity.


Certificates, Licenses, Registrations

* Valid driver’s license in accordance with HCI motor vehicle policy.
* Maintain current Crisis Prevention Intervention (CPI) certification.
* Maintain current American Red Cross CPR/First Aid certification.


Physical Demands

The employee routinely communicates in person or by telephone, travels to consumer locations, stands, walks, reaches, bends, uses hands, and may lift or move up to 40 pounds. Occasionally the employee sits.


Work Environment

Subject to change depending on scheduled location. Conditions vary from hospital, residential, to community settings.

* 40+ hour work week.
* Overtime rate after 40 working hours.
* On‑call schedule to support crisis.
* Nights and weekends periodically.


Conditions of Employment

* Satisfactory reference and background investigation checks.
* Completion of pre‑employment drug screening and ongoing drug/alcohol tests upon reasonable suspicion.
* Completion of comprehensive background checks (criminal fingerprinting, Indiana State criminal history, etc.) at hire and every four years.
* Fingerprints required.
* Proficiency in computer use via pre‑employment testing may be required.
* Completion of tuberculin screening within three days of employment and annually thereafter.
* Completion of MMR, Varicella, influenza, and coronavirus vaccines.
* Completion of center‑wide new employee orientation and all required paperwork prior to reporting to work.
* Completion of Recovery Works Training, DCS Training, HCI Quality Training, Trauma‑Informed Care, e‑Learning, CPI Training, CPR/First Aid Training, and annual restraint training (for IPU staff only).
* Completion of HCI OBHP training modules within 90 days of hire or transfer.
* Attendance at all mandatory staff development and training.
* Successful completion of EMR orientation/training within the first 30 days.
* Successful completion of initial competency assessment within the first 30 days.
* Successful completion of new employee department checklist within 90 days.
* Successful completion of a six‑month on‑job orientation period.
* Successful completion of annual PES.
* Adherence to all Hamilton Center, Inc. policies, procedures, rules, and regulations.
* Participation in payroll electronic deposit.
* Adherence to Compliance Program Plan.
